Lost Property Report
File a report for a lost item.
General Assistance Request
Request general technical assistance.
The following forms require you to login before you can use them. When you click on the request link, you will be asked to authenticate. Use your Engineering login ID and password or your Iowa domain/HawkID (iowa\your ID) to authenticate. If you do not have an Engineering account or a Hawk ID and want to request one of these services, please call the CSS consultant, 319-335-5055.

Video Conference Request
First request the room from the SC building manager. Then fill out a Video Conference Request.
File Restore Request
Request to restore deleted or overwritten files.
Henry Electronic Classroom (1245 SC) Request
Request use of 1245 SC for course, TA hours, teaching/training session.
Laptop Reservation Request
Request the Linux laptops for use with classes taught in 2217, 2229, and 3505 SC.
Microsoft Campus Software License Request
Request license for Microsoft campus agreement software.
Network Connection
Request a new network connection or to move a computer from one computer jack to another one. This is the form to be used for displaced faculty and staff from the floods.
Server Request Form
Request that a computer be allowed to operate as a server on the Engineering network.
Request a New CCAD Engineering Account
CCAD administrators may request a new engineering account.
